I’d like to have my medical records transferred to a new dentist that’s not a 42 North Dental supported practice. What do I need to do?
If you need to transfer your records, call our office phone number directly and your call will be forwarded to a patient care representative who will walk you through requesting a records release. There will be no charge for this service. You can also be assured that we keep all records securely stored for 8 years as required and you can request them at any time. There is no immediate rush or concern.
Do I need to transfer my x-rays and records to Four Town Dental or another Gentle Dental office?
Your records, x-rays, and treatment plans will be automatically available at Four Town Dental or any other Gentle Dental location. There’s no additional action you need to take.
What if Four Town Dental is not a convenient location for me?
